What Is Myir Stand For?
Known as the Vaccine Information System (IIS), the state’s immunisation register, MyIR (short for “My Immunization Records”) keeps your medical history current (IIS). Besides your COVID-19 vaccination record, it also keeps a record of all your family’s immunizations (for dependents under the age of 18). It also provides parents and caregivers with quick and easy access to a kid’s immunisation status for school, sports, and child care registrations, as well. MyIR has been around for a while, and now it’s much better! Washington is one of the states that has used it for years.
Myir Registration Create?
To get started, go to www.MyIRmobile.com and create a new user account. A list of possible record types will be presented to you. Due to a recent change in MyIR, you may have to re-register in order to view your school records. Please keep in mind that each adult over the age of 18 will have to register their own account. A unique email address is required for each new account (one email address cannot be used to register for multiple accounts).
Step 1 : You will receive a verification code by text message or phone call if you selected “Auto Match” and the system found an exact match. Your family can then be added to MyIR via a simple on-screen prompting process.
Step 2: Fill out an Authorization to Release Immunization Data (PDF) and mark the box stating that you want to obtain your records online via MyIR (the Immunization Records Request System). Then send it to DOH by mail, fax, or e-mail at:

- To register, select
- Select Mississippi from the drop-down menu.
- Create a password, then enter your e-mail address and first and last name.
- Afterward, you’ll need to validate your account by phone or e-mail by clicking Sign Up.
Immunization Records Where Can I Find Them?
- You can access your records at any time after creating an account.
- Select Find My Records.
- To help the system locate your and your family’s immunisation records, fill out the relevant personal information.
- You can access your immunisation history, immunisation needs, and a Form 121 by selecting Take Me to My Account if your information matches an item in the immunisation register.
- Use the green chat bubble on the bottom right of the screen to provide more details if your information does not exactly match an official immunisation record.
You Can Tell Us How You Like To Be Contacted.
You accept to receive electronic communications from us by signing up for myIR. If you don’t want to receive print statements, notices or other correspondence from us, you can opt out. In place of this, we’ll notify you via myIR whenever you get a mail or secure message. You have the option of being alerted by:

Your myIR account can be activated by sending a text message or by contacting our customer service department.
In order to activate your account via text message, you’ll first need to submit your cell phone number. To activate your account, you’ll receive a text message with an activation code. We are able to accept mobile phone numbers from the following countries for international use

There Are A Number Of Ways To Obtain A Copy Of My Immunisation History.
The Immunization Information System (or other electronic healthcare records) is used by most health care providers in Washington state to keep track of your immunisation history. Requesting a comprehensive immunisation record from your doctor or pharmacist is a simple process.
An authorization to release immunisation records (PDF) can be emailed to WAIISrecords@doh.wa.gov requesting a copy of your records to be delivered to you through e-mail, postal mail, or fax at no cost if you can’t acquire a complete vaccination record from your doctor. Alternatively, you can print and complete the form, then send or fax it to the address or number listed in Step 2. Requests for immunisation records typically arrive within 5–8 business days of being submitted.
Request A Full Vaccination History From The Department Of Health.
To acquire a complete immunisation record from your provider, contact the Office of Immunization and Child Profile at 360-236-3595 or 1-866-397-0337 if you don’t want to sign up for MyIR. The Washington State Immunization Information System is used by the Department of Health, but not all people’s immunisation data are included. Your name, date of birth, and current address will be required when you call. Only the existence of a record in our system will be revealed at that time; no other information will be divulged over the phone. This is to ensure that your personal information is safe and secure.
