How To Myuclahealth Login
- Patients can request an activation token on our website (myUCLAhealth.org), however this option is not yet available on our MyChart mobile app. This feature is only available to patients who are at least 18 years old.
- Click “Sign Up Now” on the website’s login page, then “Sign Up Online” under “No Activation Code?”.
- Our third-party verification system will be linked directly to this form, so please complete out the relevant information.
- If you sign up for myUCLAhealth, you’ll get an activation code through e-mail or text message within 2-4 business days.
- You must sign up for myUCLAhealth in person with photo identification if you are a new patient

Patients can now join up for myUCLAhealth via Instant Activation from our clinics, thanks to the rapid expansion of Telehealth made possible by COVID-19. How Can I Get A Copy Of My Child’s Or Loved One’s Medical Records Through A Third Party?
We urge that you seek help from your doctor.
Proxy access can only be granted in person under regular conditions. As a result of the COVID-19 regulations, we are making several exceptions. You may be able to get access to your doctor’s office via proxy over the phone for a limited time, provided certain conditions are met.
The age range of children who are exempt from signing a consent form is 0-17 years.
For children between 12 and 17 (which is considered an adolescent in the state of California), the teen’s verbal agreement must be secured before proxy access can be granted.
For teenagers aged 12-17, proxy access to the medical record (i.e., health issues, allergies, immunizations, and test results) will be restricted. They will not have access to past or future appointments, hospital admissions, letters, referrals, clinical notes, or medication/refill requests. Because of this, we recommend that parents allow their teens to have their own myUCLAhealth accounts in order for a video visit to be possible.

Proxy Access To An Adult Patient’s Medical Records Is Available Through What Means?
- The doctor’s office is prepared to meet with the patient and go over the details of proxy access if the patient completes a consent form.
- Keep in mind our new Proxy Invite option. Patients can now grant complete access to their medical records to a trusted friend or family member. A myUCLAhealth patient account is required for both parties. Invite someone by following these simple steps:
- Select ‘Your Menu’ from the drop-down menu that appears.
- Go to Sharing.
- Share My Record.
- Once you’ve entered all the necessary information, click Send.
- An email with an invitation will be sent to the person you’ve invited. An invitee must submit their own date of birth before being granted access to a patient’s account.
