MyUPMC Children’s Health Proxy
Pediatric Proxy enables parents and legal guardians access to their child’s medical records through MyUPMC. Parents and legal guardians can use Pediatric Proxy from the time of their child’s birth until the age of eighteen.
How Myupmc Members Can Receive A Pediatric Proxy:
- Using MyUPMC, go to “Manage Another Family Member’s Account” and fill out the online request form for a Parental Power of Attorney for Healthcare.
- Children’s Community Pediatrics will send you an email confirming your request for access to your child’s medical record after it has been processed and authorised.
- Log in to MyUPMC and choose the child’s name from the dropdown menu to activate your Pediatric Proxy account.
- To proceed, simply click continue after reading the notice about the proxy.
- Click on “Review Your Messages” under the child’s account once you’ve logged in. Any menu choice on your child’s account will activate your proxy access.
